Acute agranulocytosis during therapy with an analgesic containing dixyrazine

Agranulocytosis developed in a 72-yr-old patient after 2 wk of treatment with tablets of Algolysin-Forte, 3 times a day, for severe osteoarthritis of the hips. The patient had fever, prostration, dysphagia (probably related to transient esophageal ulceration) and profound leukopenia which subsequently abated on withdrawal of the drug. The patient received other potentially toxic medications but in vitro tests confirmed Algolysin Forte as the responsible agent. This preparation includes paracetamol, propoxyphene, codeine and dixyrazine, the last of which was implicated as the toxic agent in this case. This is the first report of this side effect of dixyrazine.
