Adenocarcinoma complication Barrett's esophagus: report of 5 cases

Five cases of primary esophageal adenocarcinoma arising in a columnar-lined (Barrett's) esophagus are reported. A history of gastroesophageal reflux was present in 2 of the 5 patients. In 1 patient, the carcinoma was limited to the submucosa, but extended for 6 cm over the surface. In 2 other cases, the diagnosis of a malignant change required systematic biopsies. An area of specialized columnar type epithelium in the Barrett's esophagus was observed in all cases. Barrett's esophagus should be considered as a premalignant state and surgery should be considered in the presence of marked dysplasia.
