A 43-yr-old woman was admitted with a several month history of epigastric pain. She was diagnosed radiologically and endoscopically as having a leiomyosarcoma in the duodenum. Laparotomy revealed many leiomyomatous lesions in the duodenum and in the stomach and jejunum. Pathological examination proved that all were leiomyosarcomas showing many mitotic figures (mitosis index 2.0). Many microleiomyomas and microleiomyosarcomas, which were found microscopically, were seen in the muscle layer of the stomach. This paper reports a very rare case of leiomyosarcomatosis and reviews of the relevant literature with regard to its incidence, pathology, diagnosis, treatment and prognosis.
