Post-traumatic myoclonic syndrome. Effectiveness of the thalamic lesions on the action myoclonus

A case of post-traumatic action myoclonus associated with palatal myoclonus and cerebellar syndrome submitted to stereotaxic thalamotomy is reported. The electrical stimulation of the posterior part of the ventrolateral nucleus was followed neither for clinical nor cortical responses. The cryogenic inhibition resulted in an almost complete blocking of the myoclonic jerks. No influence was registered upon the palatal myoclonus. Post-traumatic myoclonus has rarely been reported. In a review of the literature no such patients were treated surgically. Physiopathological mechanisms involving both types of myoclonus are discussed.
