A simple, non-invasive, quick and reliable single-dilution shake test was performed on gastric aspirate samples from 250 babies (59 preterm and 191 term). Hyaline membrane disease was correctly predicted in 9 of 11 preterm babies with a negative shake test. Of the 45 preterm infants with a positive shake test, 1 developed idiopathic respiratory distress syndrome. None of the preterm infants with intermediate positive shake test developed respiratory distress syndrome. Of the 191 term infants, 4 (2%) had false negative shake test. Shake test appears to be a quick and reliable predictor of the development of hyaline membrane disease, and it can serve as an additional diagnostic tool to support the clinical diagnosis of hyaline membrane disease.
