Paroxysmal ventricular tachycardia in a patient with post-traumatic left-ventricular aneurysm, and its treatment

As a result of an accident, a foreign body penetrated a man's chest with a particle of the foreign body lodging in the interventricular septum. The particle was later removed by surgery. Nine years later the man complained of daily occurrences of paroxysmal tachycardia with severe chest pains. He was treated with antiarrhythmic drugs, the beneficial effect of which was temporary and only electric impulse treatments (104) effected any improvements. X-ray, coronarographic and left ventrilographic data revealed a posttraumatic calcified aneurysm of the left ventricle and paroxysmal ventricular tachycardia. Drug therapy or the use of .beta.-adrenergic receptor blockers, competitive ventricular stimulation or use of external asynchronous stimulator with low doses of antiarrhythmic drugs were ineffective or only temporarily effective. Electrical impulse therapy did not effect a stable sinus rhythm. Aneurectomy was performed and had a lasting effect. Surgical procedure was recommended only for cases where medical and electric therapy were ineffective in the treatment of paroxysmal tachycardia.
