Electromyographic shimmer in patients with neuropathy and neurogenic atrophy

Electromyographic shimmer, the mean consecutive difference in the amplitude of the motor unit action potential (MUAP), was determined in 5 patients with S1 radiculopathy. It was also determined in 5 patients with traumatic sciatic nerve injury at the hip and subsequent neurogenic atrophy of the medial gastrocnemius muscle. The group of patients with radiculopathy had shimmer index (SI) values of 2.7 .+-. 0.5 units as against control values of 1.6 .+-. 0.39 units. The group with neurogenic atrophy had a mean SI of 0.5 .+-. 0.1 units. Shimmer may be used to characterize random variations in amplitude of the MUAP. It is inversely related to the amount of neurogenic atrophy and directly related to the potential for nerve regeneration.
