The exercise test in coronary disease patients with ischemic electrocardiograms at rest

The diagnostic accuracy of the exercise test in 115 coronary artery disease (CAD) patients with ECG abnormalities of ST segment at rest was compared with a group of 58 CAD patients with normal resting ECG. The exercise test was positive in 81% of patients with normal resting ECG (group 1) and in nearly all the patients with abnormalities of ST segment at rest (group 2). In the group 2 the incidence of chest pain was generally lower. A greater incidence of chest pain and a greater severity of Gensini index was found in the subjects with positive ECG response in leads different from the ones found in resting ECG. The importance of exercise ECG changes location and of chest pain appearance during exercise is emphasized.
