Measurement of gastric mucosal blood flow in man

In 5 normal subjects and 5 patients with duodenal ulcer, the clearance of [14C]aminopyrine from plasma into gastric juice was studied during the i.v. infusion of stepwise increasing doses of pentagastrin ranging from 25-2000 ng/kg-h. Acid secretion in response to pentagastrin and clearance of aminopyrine was significantly greater in duodenal ulcer patients than in normal subjects. In both duodenal ulcer patients and normal persons there was a significant correlation between rate of acid secretion and clearance of [14C]aminopyrine, and the slopes and intercepts of the lines relating these 2 variables did not differ significantly between the 2 groups. R, the ratio of gastric juice to plasma aminopyrine concentrations, which also is a measure of the ml of blood flow for each ml of juice secreted, showed a negative correlation with rate of acid secretion in both duodenal ulcer patients and normal subjects and the slopes and intercepts of the lines relating R to acid secretion did not differ significantly between the 2 groups. The relation between acid secretion and clearance of [14C]aminopyrine in man is similar to that found in earlier studies in dog, a species in which clearance of aminopyrine was a valid measure of mucosal blood flow. Clearance of [14C]aminopyrine from plasma into gastric juice may be a valid measure of mucosal blood flow in man, and that the relationship between rate of acid secretion and clearance of [14C]aminopyrine is similar in normal subjects and in patients with duodenal ulcer.
