Two cases of carotid sinus syndrome with peculiar clinical and nosological characteristics are presented. The clinical symptoms were syncope with bradycardia and hypotension, which occurred together or independently. The best medical therapy to prevent or treat the syncopal attacks was sympathomimetic drugs (high dosage dopamine, particularly) with demand pacemaker. Cessation of symptoms was obtained only after surgery: denervation of carotid sinus in 1 case, intracranial section of the glossopharyngeal nerve and of the 2 vagal terminations nearer to IX in the other case. The pathogenetic importance of stimulating the afferent nerve-fibers of the glossopharyngeal nerve is indicated.
