Renal tubular acidosis as a complication of jejuno-ileostomy in obese patients

Of 22 patients treated by jejunoileostomy for extreme obesity, 17 were examined 1 to 9 years later. One had permanent renal tubular acidosis 4 years after surgery. The 16 others were given, when fasting, ammonium chloride 0.1 g/kg bodyweight, usually by vein, and urine pH was studied for 5 to 8 h. All developed acidosis. Of the 6 incapable of adequate acidification of urine, 5 showed slight spontaneous reduction of standard bicarbonate and 2 had slightly reduced serum K.
