Ductal manipulation--a note of caution

Increasing numbers of premature newborn infants with patent ductus arteriosus are being hospitalized. In some, persisting ductal patency with left-to-right shunting jeopardizes a favorable outcome. The effect of early ligation is questionable, with some infants having positive effects but others who might have fared equally well without surgery. Since prostaglandins affect the tone of the ductus, these have been investigated, but the results are inconclusive. The possible beneficial effect of indomethacin-induced ductal closure in the management of premature infants who suffer ill persistent patent ductus has been reported, but caution must be exercised to avoid possible toxic effects.
