Hyaline membrane disease (HMD) occurred more frequently in 2nd-born twins and contributed to the known increased perinatal mortality and morbidity associated with the 2nd twin. Fetal lung immaturity as well as intrapartum asphyxia were 2 known separate causes of HMD. An attempt was made to elucidate the part played by each factor in the etiology of HMD in the 2nd twin by using ultrasound to produce a safe, reliable method of separate puncture of fetal sacs. Several cases of complicated multiple pregnancy were described in which this technique was used.
