Pieces of the mycardium of the right and left auricules and papillary muscles of the left ventricle taken during surgery in 23 rheumatic patients were used to study myocardial ultrastructure. Changes in 5 cardiomyocytic organelles (nucleus, mitochondria, myofibrils, sarcoplasmic reticulum and sarcoplasm were evaluated. Central hemodynamics were studied with isotope cardiography using human albumin serum labeled with 131I and echocardiographic data. EM data were compared with indices of central hemodynamics, duration of decompensation, stages of circulatory disturbances, rheumatic process activity and length of rheumatic history. The effects of prolonged damage to the circulation by rheumatic disease were observed as destructive changes in cardiomyocytic organelles and led to considerable damage to contractile functions of the myocardium.
